For: The Community Acute Services will cater for those individuals who have acute mental health needs as an alternative to hospital admission and to assist with early discharge from an inpatient facility.

Referral methods: GP, CMHT, Inpatient Services and A&E Departments

Open: Monday, Tuesday, Wednesday, Thursday, Friday, Saturday, Sunday

Bank Holidays 9am - 5pm. Acute Day Hospitals 9am - 5pm. Crisis Resolution and Home Treatment teams 24hrs.

Description: The Community Acute Service will provide a realistic alternative to hospital admission for persons experiencing acute mental health problems as well as helping to facilitate early discharge from inpatient facilities. Those who do not require 24 hour medical and nursing care can be referred for prompt assessment and treatment. In addition the service will work in partnership with the Crisis Resolution & Home Treatment Teams providing a rapid assessment for people experiencing a mental health crisis. A short term therapeutic milieu will prevail through the provision of an active, varied and structured group programme. Long term treatment options and needs will be indentified prior to discharge in conjunction with others as part of the CPA process. NB. Word document needs to have address,email & web icons removed.
