Our Community Navigators aim to help you access services and groups in the community to meet your social, emotional, medical and practical needs.

Who is this suitable for?

Anyone over the age of 18 and who has been referred by their GP or other Primary care NHS professionals. We are part of the Gateway Team at Oxford Health NHS Foundation Trust, covering Buckinghamshire.

What can I expect?

Following a referral from the Community Mental Health Team (CMHT) from your GP, you may be allocated a Navigator. Navigators can only accept referrals from within this referral route.

Community Navigators work with you to support your social, emotional, medical and practical needs. You will then meet weekly or fortnightly for 4 to 6 weeks. One-to-one meetings will either be online via video link or within a suitable NHS or community premises. Your Community Navigator will discuss with you what type of meeting and location is best for you.

Your Community Navigator will work with you to identify community support that will help meet your needs, make referrals on your behalf and help you to access services and community groups.

Community Navigators are approachable, empathic and flexible. They will listen to you and help you identify coping strategies and manageable goals.

Fees

There is no charge for this service.

How can I access this service?

Your GP may refer you to the Community Mental Health Team. The first point for anyone experiencing any mental health problems should be your GP, who can then make a referral to the CMHT.
